Transcend's packages have changed per Colorado law. According to the current state policies, ALL clients are required to complete the following intake forms to participate in ALL of our services.
Thank you for trusting the Transcend Team with your care. To receive harm reduction services, you must:

Be over age 21

Not be pregnant
-
Not be taking ssri’s, seizure, stroke,or heart medication
-
Have no Mental Illnesses with active psychosis as a symptom
-
Have no suicidal ideation for the past 6 months
-
Have no alcohol or other drugs while taking psilocybin
-
Have no physical or sexual contact with others while taking psilocybin.
-
Have an emergency contact who knows you are taking psilocybin.
-
Take psilocybin at home or another safe, private place.
-
Not operate heavy machinery.
-
Have an emergency contact.

Are legacy, certified, or licensed Psilocybin Facilitators who serve clients but do not have a trusted source for medicines.​​

One-on-one 30-minute consultation regarding the facilitator's client needs and how to best address them (In person, phone or via Zoom).
